My question is what type of Caddis is this? And what does the adult look like.

All species were found in Wellman Lake Duck Mountain.

Would the caddis be a Hydroptilidae Hydroptila?

 

Wellman-

 

Probably not Hydroptilidae; more likely Rhyacophilidae. However, given the lack of resolution in your photo, determining identification from it would be virtually impossible.
