Thank you @niveker. It's an Umpqua "U" series 002 traditional, standard dry fly hook.  In fact I really like the "U" series hooks. They're about half the price of the Tiemco's and I think they ate just as good quality wise.

Melted Cased Caddis
 
Hook - Mustad 3906B or equivalent
Weight - Lead free wire (Optional)
Thread - Chartreuse and black
Case - Root beer pearl chenille or crystal chenille
Legs - Black hackle fibers
Head - Chartreuse and black thread
UV Resin
 
Tying Note
 
The chenille is tied on at the hook bend, then wrapped forward and tied off. Cut off thread.
The chenille is then melted using a lighter or other flame to create the case. 
Reattach tying thread and finish the fly
